Work Experience

Consultant, Tiffany Sosnowski Consulting | 2022+

  • Lead program evaluator for Mentorship pilot project within Fraser Health Authority

  • Agency-wide workforce training with St. Leonard’s Youth & Family Services Society (3-part staff training on substance use, harm reduction and youth engagement)

  • Subject matter expert contributor and advisor with Provincial Trauma and Resiliency-Informed Practice Initiative

  • Oversee program responsibilities including committee membership, program development, governance, faculty workload & education plan, and day-to-day operations

  • Concurrent Disorders Clinician with the Youth Integrated Case Management Program

  • Offered foundational support and expertise to the launching of this new program within the health authority

  • Provided program coordination and clinical staff supervision for the Short-Term Assessment and Response Team

  • Lead the interdisciplinary team through ongoing program change and re-design

 

Clinician & Case Manager, Fraser Health Authority | 2008-2017

  • Primary Clinician and Case Manager for the Creekside Youth Withdrawal Management Program

  • Developed program: the first youth-only medical withdrawal management program in the province

  • Conceptualized, collaborated with community partners, and implemented the 30-day youth stabilization program within Creekside withdrawal management
